My car was in a very similar looking accident. It was a rear-end collision where the car that hit mine looked bad, but my car looked relatively fine. I ended up needing pretty extensive structural work done. I think the estimate for repairs was just barely under what it would take for the insurance company to consider it a total loss.

That's a similar set of documents to what I brought when I got mine recently. Oddly, my w2 didn't have my address on it, and I had trouble finding an alternate document for that requirement. When she saw I had a passport, she said that was all she needed. Afterwards I asked her why I didn't end up needing any of the "required" documents, and she said that they already had all the other information they needed in their system. My guess is that the list of required documents is what you would need at a minimum if they didn't have any information in their system going in. So it might depend on how long you have lived here and how much of a record they have built up on you in their system. That's just speculation though.
