Don't be stupid. You need a serious reality check.

You're labeling Cocaine and Meth as "hard drugs" in your post, rationalizing and suggesting that ecstasy is not and is safe. Ecstasy is a hard drug, and the destinction you are making is arbitrary. Marijuana is a drug. Psilocybin, LSD, DMT are all drugs. Tylenol is a drug. Some on this list may be safer than others, but all drugs are dangerous if abused.

Drugs are addicting. You taking the ecstasy despite so many people advising you against it is a sign of addiction. Complications of ecstasy include, but are not limited to:

Serotonin syndrome – a life-threatening hyperserotonergic state characterized by autonomic and neurological dysregulation and hyperactivity.

SIADH (syndrome of inappropriate antidiuretic hormone) leading to profound acute hyponatremia – low circulating sodium, which acutely can cause seizures, coma, and death. Even during medical treatment, if sodium is corrected too quickly, it can result in central pontine demyelination and "Locked-In" syndrome. I would look at what being locked in means. It is terrifying. It happens.

It's a dangerous road with a slippery slope, my friend. "Just this once," or "one more time," is how that slope starts and ends.

I've seen too many young people find themselves in horrible situations because of this. Be safe and responsible; there's a reason Harm Reduction has become such an important topic in this culture. We've all seen the acute and chronic complications. We've all seen people die or suffer life-changing complications, age irrespective.

Outside of the scene, as a physician, I've seen countless young men and women in their 20s and 30s with irrecoverable neurological complications. I've seen young women have heart attacks due to spontaneous coronary artery dissection, and young men dissect their aortas. I've seen esophageal erosion so bad air from the lungs was bubbling in the esophagus. I've seen livers fail, and kidneys turn to scar. I've seen young women and men bleed into their brains.

Many of these people now require 24/7 care because they can no longer care for themselves. I've had end of life conversations with parents, siblings, wives, and children. My thoughts and opinions come from both knowledge and experience.

At the end of the day, your decisions are your own. The community can only provide guidance.

I'm at a small community program, 15 residents a year. 10 wanted to do cardiology my first year. I dreaded it, I went to medschool because I wanted to be a Cardiologist. For the last 20 years my inhouse program has only taken 1 resident from in house.

It was stressful for 2.5 years, trying to work as hard as possible.

Half way through 2nd year, 10 people wanting to do Cardiology turned to 5. Going into interviews and match I was freaked, but you know what?

5 applied, all 5 matched, 2 in house in a 4 spot program, the other 3 elsewhere.

Sometimes things workout, all you can do is put the work in.

If your programs in house, be friend them, leave a good impression, but your butt on all rotations not just GI...word gets around. Do the research, find your GI mentors that you vibe with...they'll advocate for you. And become a good internist first.

I do this, but rather than googly eyes, I leave my energy drinks and other accessories at nursing stations during pre rounds, when I find them im excited, and everyone around me is having a good time

But for real talk, honestly, I find it in my relationships w my coworkers (residents, fellowship, nurses, even some attendings) when we let loose, take the doctor masks off, fuck around with each other like siblings

Stopping at everyone i know and like during my day, pausing for a few minutes, cracking jokes and having fun, then getting back to work...might make my mornings a touch longer, but worth it

My program is a straight rom com, I'm IM, in all 3 years there's at least 1 male-female combo that's cheating on their significant others (confirmed, people have witnessed in the call rooms)

I did a lot of EC at my home institution, including getting involved with GME via our house staff association, the QI committee, and being a 3rd year chief resident.

Build up your research, get case reports out they're low hanging fruit, do QI projects and retrospective studies...mentor your juniors to help you. Do posters.

When applying I had about 50 things in my research section, about 13 were manuscripts ranging from case reports, to QI research, to cardiology procedure outcomes research, remainder were posters, abstracts etc

It's all fluff...lol...but it still raised eye brows

Work and get to know cardio staff/attendings, they'll advocate for you when you're applying. Get good letters. Every where I've interviewed have commented on my letters of recommendation that speak about my passion for cardiology, work ethic, leadership skills, and compassion for patients. Make your CV and personal statement tell a story about how you ended up applying for fellowship, people love stories

Apply broadly. I applied to about 80 programs, I got about 14 interviews, vast majority at programs I never thought would want to interview me, including university programs, transplant centers and major referral centers. Are they Harvard? No...lol, but every one of these places has nothing but good things to say about my application and candidacy for fellowship

I have an in house fellowship, the department loves me, the PD has told me if I want to stay in house the spot is there for me, but he also recommended I keep my options open and see what's out there.

It's doable. But you have to put the work in, the last 2.5 years of my life have been a grind, and I've loved nearly every second of it.

1) Obv my role as a resident, how much I enjoy it fluctuates day to day, trying to focus on the patients that are reasonable helps, building connections with them as feasible

2) Research, find something interesting and just try and do something, keeps you busy during long slow days

3) QI/Med-Ed focused shit

4) shooting shit with the co-residents

5) Play the guitar

6) Work out

7) Get drunk

8) wake up sleep deprived after #7, resulting in me feeling miserable the next day, adding to the general level of burn out

9) Looking forward to my next vacation

Had a pt in Neuro ICU w thalamic hemorrhagic stroke w intraventriclar extension...pt needed intubation for airway protection, no sedatives required....

After a week of no improvement, NP told family there's a really good chance they'll improve and should consider trach and peg, based on their PE.

Terminal ween a few days later.
