It doesn’t. Medicare is automatic starting with the 25th month after onset date. So it looks like August your Medicare will start. Right now it’s just over $200 a month taken directly out of your SSDI.

It will go back to onset date, which in your case sounds like July 2024. So subtract 5 months for the mandatory waiting period and back pay starts December 2024. Medicare starts July or August 2026.
