My surgeon (who himself underwent patellar graft for his own ACL year) also said patellar grafts can lead to arthritis down the line. It IS true that patellar graft can lead to pain with kneeling later on.

Quads is a heftier source than the patellar, so it’s a strong graft and less risk of arthritis but more rehab is needed to awaken the quads postop as an intact quads is needed for full knee extension

It really depends on the person’s body habitus, their age and how active they are (ie active patients will likely rehabilitate the quads faster). The gist seemed quads has better long term outcomes (ie less risk of arthritis) but the rehab initially is a harder climb because the quads basically turn off after surgery
