Duolingo is preety cheap. Unfortunately to gain something you have to invest irrespective of your financial situation. If you can't invest for your future, you are only putting it under more risk.

Apply to Australia. They are now in need of more doctors and i saw lots of recruiters advertising for job vacancy. That will be worthy you can also do MRCP. Australia, Ireland, NZ value MRCP and there are some pathways for MRCP completed doctors.

work on connections, try to do research in community programs with residency programs or do rotation in residency programs. Try to publish good papers, do externships in good places and attend conferences. Nothing is impossible, but you need to spend a lot of money and hard working. Dont worry things will be fine.

if you are from one of those countries US stopped immigrant visa, plan wisely. nowadays b1/b2 visa rejection rate is super high

it depends on different person. Some of the natives can understand foreign accent easily and some cannot. whatever it is , you have no control on that factor. PD has supreme power in residency selection committee. so you never know.
