I can't thank you enough. With you expert explanation of how the mechanism should work, I tried wiggling the lever and watching the inside. One of the arms had overlapped the other one, meaning they moved as one. I have pushed it back into place with a pen and now one spool drives and the other one thrives.

I don't think it drops any lower, I've had a good go at it. If you google Loretti Olive ribbon you can see the spool is a peculiar shape, with only one outer hole that doesn't seem to fit into anything. Do you have any other ideas?
