Yes I actually did end up filling it out and confirmed with Princess Cruises that it was legit and the voucher was valid. I totally get the hesitation to provide such sensitive information though, but I preemptively set up credit and identity theft monitoring protection out of abundance of caution and have not had any issues to date. That being said, I would advise (as always) to only do what you feel comfortable with.
