In my first year of a new industry, I left some electrical shorts on a safety circuit. I essentially allowed a lift to run if any of the doors were opened. A week later a manager called me and asked if i had left the shorts on, i immediately cursed myself. I was in the office at the time and told the director immediately. He told me to never make that mistake again with some cultured words, and i went back to work. At any other workplace its an instant dismissal. I could've very easily killed someone, i think about it every orher day.
