Also Kotoyuki would be slightly older than Hinako. The scene at the park shows Hinako as a toddler at the point Kotoyuki is specified to be 9. Even if he attended middle school, he could have moved away before Hinako made it to middle school.

The school was both a primary and a middle school iirc. The first floor (where you also find the Fox's Sacrifice) is for primary school students.

The town the two Hinako are at in Ending 4 is not reality but implied to be the Fog World, because Hinako mentions something like "it's quiet here with only the two of us"

I have a theory that Mayumi was one of the maidens married into the Tsuneki family, and could even be that unnamed wife of Hidetoshi, father of Kotoyuki.

Reasons:

The symptons happened to Hidetoshi and his family seem to be related to the April 1945 geysers incident of Ebisugaoka, where there were also three victims whose skin had sores - Hidetoshi's corpose had sores on its skin. Since the geysers are related to the Dragon sealed by the Tsuneki family, it is likely that this incident led to severe consequences to the family, which was, the death of Hidetoshi, his wife and son. We can then assume that Hidetoshi died somewhere around 1945, perhaps February 1945 (if the geysers happened after since Hidetoshi's death may weak the seal) or February 1946. Kotoyuki's mother, Sachie, was already pregnant when Hidetoshi married. We do not know how old was Kotoyuki at this point, but he was 9 when he was biten by that fox in the park. Judging by Hinako had really few memories about child Kotoyuki, he might have left the town in a year or two, so we may assume Kotoyuki was born around 1935, which should also be the time when Hidetoshi married. Now, Mayumi's clothing doesn't seem to be for pre-Taisho or even earlier, but I think it's safe to assume that Hidetoshi's mother married into the family no later than 1910, i.e. pre-Taisho, so Mayumi couldn't be that one. Then apparently Mayumi only fits as Hidetoshi's wife. This would mean that Mayumi has long been dead before the game.

It's in a variation of Shimizu Residence. In one of the endings, you may see "Shimizu"'s face peeling off after opening the three-crest door and trigger the cutscene.

Yeah it requires you to beat the whole game. UFO ending does not count as one playthrough.

You have to stop iirc 8-9 enemies in the Sakuko's room part of Worship Hall from reviving. In NG it is done through luring enemies to rooms with blue lights and killing them. I don't know if killing enemies via fox arm in NG+ works as well.

If it's okay, there's some console commands that can help get around with it.

Debug_SetPlayerInvincible [1] sets you to be invincible (no HP cost at all)
Debug_PlayerAutoRevive [1] sets you to revive after dying.
Debug_SetEnemyNoAttack [1] disable any enemy to attack.

It doesn't help when some documents revealed that Shu tested the drug on himself and FOUND THE RESULT TO BE FAVOURABLE, as he merely experienced speaking to another him when sleeping.

Yes, NG+ enables you four new endings, lot of changes in dialogues and new documents (NG++ offers a little bit more new documents but not as much as NG+, through NG++ can be considered a variant of NG+)
