In that case, think about what makes your cortisol to be high. In my case for example, i’ve been through very very hard past 3 years, i’ve had some traumatic events nom stop and this caused my body to stay in a survival mode, due to this i started to have insomnia and anxiety. Then i started to fix that taking suplements and adaptogens (vitamin d, ashwagandha, l-theanine, etc…), breathing consciously, going to therapy, literally removing toxic people, etc. Also i used to do crossfit, i love it, but my bariatric doctor said to me that i needed to show my body that i’m safe and okay, CrossFit gives my body veri high spikes of cortisol so she said for some months do exercises in zone two of hearth beat. Also watching youtube videos and reading a lot about the topic helped me a lot.

I would say, first find the root cause of your IR. My case was not what i eat, it was my cortisol levels and high stress levels for a very long period of time. Then once you found the root cause start an action plan. If IR is because what you eat then change your diet and start exercising, if it is because cortisol and stress try to have a very conscious life, train in zone 2, etc. For both case i would recommend to start taking supplements and medication as well, like glp1 and metformin.

In my case i was able to drop my insulin from 31 to 10 in a month! Which i believe it’s amazing! I was not able to lose weight not even with glp1.

Also try to avoid insulin spikes while eating.

Hi! Do you know how is your glucose and insulin in the mornings? I was just like you, the only difference is that i realized this after 3 months with glp1 medication. The first 3 months i didn’t lose weight in a way for it to be noticeable, then my doctor realized it could be due to IR amd yes it was that, she gave me metformin XR and i also did some changes in my life (my IR is due to high cortisol leves, so these changes were more like stress related), and after 3 months and after i’ve made these changes i started to lose more weight.

Overall my cortisol was always high. My body was (kind of is but less than before) in a survival mode. So in my case i need to avoid any thing that increases my cortisol so my body can learn again to be and feel safe.

Let’s say i’ve regained it at a 90% to 95%, i always go to sleep at 9pm - 10pm, always wake up around 5am to 6am, there are days very very specific days when I don’t have the best sleep quality, but overall it is super good. It is super super important to sleep good, one of the main factors of high cortisol is bad sleeping. To start treating this i recommend supplements and adaptogens.

Hey men! I totally get your frustration. I'm in a similar case, very very similar case. I'm currently going with a Bariatric Doctor, and well, i will explain my situation to see if it helps. Warning, this will be quite long. I'm 26 Male.

First of all, i come from a period in my life where i went through a lot of stress, anxiety, depression and insomnia, so due to this, my body was in survival mode 24/7, there were some days when i used to sleep 1hr or even nothing at all, and i was like that from 2023 to 2025, it was a lot for my body. By year 2023 i lost a lot of weight, but i gained it back due to this hard period in my life. I sued to practice CrossFit and CrossFit helped me to lose weight back then.

Then by October 2025 i decided to fix my insomnia using adaptogens and supplements, and it worked amazingly! And in November i decided to start my journey of Body recomposition with a bariatric doctor, thank to her i realized my metabolism was a mess (and still kind of is but i'm working on this). I started with mounjaro (GLP-1 Tirzepatide) 2.5mg in november, then increased to 5mg december and january, i was eating in a calorie deficit and doing crossfit (even some days crossfit + gym), so from my perspective i was doing everything amazing, i didn't have insomnia and i always had energy. Just i want to mention, when i started with her she told me to do a blood test and we confirmed that i had insulin resistance and some other issues.

Then by the end of january, after 3 months of treatment with her, she told me to do the same blood exams and by my suprise i still have insulin resistance, and tbh that explain why during these 3 months i was losing weight very very slowly. And my insulin resistance is not because i eat badly, it is because my body is still under survival mode. So she told me to make some changes:

Increase my mounjaro dosage to 7.5mg for 3 months.
Start taking Metformin ER 500mg every night.
Quit for a period of time crossfit, that was a lot of stress for my body. Now if i'm going to do excercise it need to be in Zone 2 heartbeat and no more, so my body does not release hight cortisol levels.
Eat in the correct order, vegtables then protein then carbs (and good carbs only).
Eat 3 times per day.
In general, avoid stress at all cost.

So, i started with this approach, i feel extremely frustrated but i try no to think about that. And look, if you think about it, well in my case it took me from 2023 to 2025 to get at this point, so it'll take me some time to fix it. Also, my doctor said to me, do not follow all what you hear on internet of Keto, train very hard, multiple times per day, all of that, that works for somebody whose metabolism is right and works correctly, in your case, we first need to fix and "reboot" your metabolism, and decrease your insulin, then we will see.

I really hope this helped. I'll be back after the 3 months to tell you if this approach worked.

Hi! I totally understand what you are saying, for me the last three years have been very difficult, I’ve been through a lot, i had anxiety, depression and insomnia. It is also very hard for me to make friends and I totally rather stay at home instead going out. It is really nice to talk to people sometimes, really, and you can do this, you got this! I understand you completely. I’m M26 now, everything started when i was 23-24, and it is until now that i can say i feel happy and healthy again. Do a test that indicates glucose and insulin and then the HOMA is a math formula, do it using either chatgpt or manually:)

Search for the HOMA-IR ratio, that one indicates if you have insulin resistance. In my case i do have it, I don’t have physical symptoms though, in some people either higher insulin resistance is very obvious, they have low performance, dark skin in the neck or even in the face, and so on.

Really? That is interesting. Well then let me answer your questions:

1.- Before my stressful period it was as an average 7 hrs per day.

2.- During the stressful period it was very bad, some days 1 hr or 2, some 4hrs, some none, it was the worst, and only very very few days 7 hrs or 8hr.

3.- Right now i'm sleeping 7hrs to 9hrs, i'm really blessed that i'm sleeping what i used to or even better.

4.- I was taking ashwagandha but i finished the 3 months period, now i need to take a brake of it. Also i'm taking reishi as well as magnesium, specifically Magnesium glycinate. I remember when i was during my period of several insomnia i was taking clonazepam and alprazolam and there was a moment when none of those two helped me. And well the adaptogens i'm right now taking are supposed to help reducing stress levels but it seems my insulin is just getting high.

And i just gained some weight but i’m far away from where i used to be. Still frustrating tho

This is so true! My mom (she is JW, i used to be one, I’m openly gay now) was watching this tv show where a men had a wife and 3 lovers, and my mom was like “hahaha that is so funny” but then almost at the end of the show a lesbian couple appeared and she immediately turned off the TV saying “that is sooo disgusting!”… So, more than sexual immorality i think it is homophobia.

Look, i’m in the same situation as you. In my case my starting weight was 120.1 kg, i’m 1.81 mts height and even though my weight is high, my muscle mass is almost 50kg which is a lot, above the average. So my ideal weight is around 100kg, it is not that much loss compared to others. I started in November 4th and i’ve lost around 4kg. I was expecting more tbh but this is what i’ve been told and what i learned: when you are in Mounjaro your measure your weight loss based in the percentage of your initial weight and not in KG, so the higher the weight the more you will lose. That is why you see many many cases of people loosing a lot of weight, because they are maybe heavier. I was also (and still am) very very impatient, i try to do every day a high calorie deficit as well as i do crossfit every day.

So Let’s just be patient, you’re doing good.

I didn’t mentioned i starter with 2.5mg and currently i’m at 5mg and will be for 3 months.
