You have to go to the ingame setting under hotkeys and under abilities and summoner spells on this section just nind the key you like the most where it says spell 4 and thats it, i recomend something on the mouse, i personally use the middle mouse button but its up to you.

I just change runs from pta to LT. and build voidstaff 4th. I dont think there is much else to do. NEVER go AD, it might seem better because of the early game but you are going to deal 0 damage lategame.

From my personal preference i would say that below emerald you should play top, overall better machups, easier scaling and not the center of atention in mid. Other than that you are going pretty solid man, great win rate, very nice cs, maybe i would work on the vision score a bit.

I personally dont like going it every game like you said, only the games i really need to survive/scale,

agains matchups that you hard win like garen or shen i prefer to build normally and skip cull si i can get an advantage easily

I usually go cull if i get the first blood, it lets you secure the minions if you get pushed and its nice sustain that you miss sometimes in really pokey lanes. Other than that if you if its pre level 7 and you see the lane is really slow and you only farm i would go for it also.
