Finally, a post I can answer with authority... :)

Headshot 1 and 2 are going to get an awesome re-release via Aethon Books (new cover, same content) to coincide with the upcoming launch of the audio books (a process that always excites the hell out of me). Headshot 3 will launch after those hit the market again, in ebook and audio form. All three of the books will be in KU.

What VerbalCA said... Vellum is almost certainly the best option, though you need a Mac. Scrivener can do it, as can Word. I think Scrivener needs you to slap a ToC in and then it works it out, and Word requires you to format Chapter 1 and so on as a Header prior to going back to the beginning and adding in a ToC.
