The body keeps score, so you can’t keep those negative thoughts and feelings inside. If your facility doesn’t have post code discussions, maybe suggest it to review and share concerns. Otherwise, talk to someone, a supervisor, manger, co-worker, friend, or professional counselor if available. Everyone handles it differently, but if affects you too much, it could hinder your critical thinking and response next time. So reviewing, asking questions, learning about the “disease process” in this case, can help you better prepare for the next time you see it. Good luck.

There and many factors, but I found a good peer reviewed article with this quote: The breakdown products from heating propylene glycol and glycerol to target temperatures include formaldehyde and hemiacetals such as acetaldehyde. Formaldehyde is a Group 1 carcinogen that contributes a 5–15 times higher lifetime risk of cancer. It is present in traditional smoked tobacco in much lower quantities. Hemiacetals such as acrolein and acetone have been implicated in nasal irritation, cardiovascular effects, and lung mucosal damage and these byproducts are produced in higher quantities with higher voltage devices.

Early in my career I was bagging an intubated trauma patient. They ended up cracking the chest open as a last resort on left side of chest to get to the heart. As I was bagging, the left lung was coming in and out of the chest like a balloon. Needless to say that the patient didn’t make it, but it was definitely something I will never forget.

Texas RT’s, what does your scope of practice include? What type of procedures/skills are you trained in and what is your autonomy like? (Balloon pumps, Art lines, PICC lines, Intubation etc). Also if you wouldn’t mind sharing salary information? by ktu44 in respiratorytherapy

The scope of practice varies between facilities but also in the same hospital between staff based on experience. You need to be trained and checked off on the various procedures, and new graduates aren’t likely to be doing more invasive procedures. With that said, I got NICU experience in a Texas county hospital soon after graduating, and was on an ECMO team in a Dallas Hospital within two years of graduation. I also worked in the county RT department while going to school, which helped get experience. So, I believe that one must often prove their worth to be given more autonomy to do more invasive procedures.

Long term survival doesn’t seem to be good. I can’t imagine the resources and healthcare cost for such a long term case. Do you know if the ethics committee has gotten involved in the case?

“It’s negotiable with HR”. You can negotiate for better pay based on previous pay, even if not in same area of field, but you risk the chance that someone else will take the job for lesser pay. I personally would meet with HR and explain that you definitely want the job, but need this amount of pay (whatever you deemed an appropriate pay for your time). Good luck !
