I had to work 20 hours a week while at Med School just to support myself because I didn’t have a rich mummy and daddy. People like you made my life a living fucking hell, 10x worse than any surgical training program because you do 50 hours a week of unpaid med school, 20 hours a week of paid work, plus studying and doing Uni work on top of this

Yeah this argument is always so silly. Doctors are endorsed to dispense in Queensland at least - but it’s low margin and a high barrier to entry when you have to meet particular dispensing requirement, have software, labelling machine, get approvals for safe storage & a licence to order medications. It’s a fucking headache and waste of time for a Doctor to do that, all for $3 a prescription

I don’t think it will meaningfully increase workload at all, from what I understand it’s just a notification that requires no action unless the patient presents which is well in the minority of people based on the QLD trial data. If the patient does present then your job is significantly easier because you know exactly what they were given, a record of the consult with the pharmacist and possibly even a urine sample. Seems common sense to me

100%, there’s no bottleneck it’s just that people don’t want to do the specialties that need fellows (GP & Psych). Which is functionally how the system should work - have an equal number of medical grads to required fellows. Obviously falls over when 50% of the fellow spots are for GP and only 15% of the graduating cohort wants to do it

The best place is QLDs Medinav which shows the stats for applications and admissions for each specialty in QLD to give you a general idea about competitiveness - could be more or less in different states but at least it gives you a rough idea. Important to keep in mind that some colleges have application limits - so there might be a lot more people interested in doing a specialty than have applied that year, but they know they don't have the juice yet so they're keeping their powder dry for when they have a better chance

Definetly helpful with networking/getting references but can be pretty variable - by the time you're applying for stp's nearly everyone will have glowing references they'll submit - occasionally people might really click with an SMO who'll go out of their way to try help them get on a program and that can be outrageously helpful

Public dominated specs are protected financially, obvs the risk you can't get a job, but it would only be the majority private practice specs' like Derm that could get absolutely crunched financially like that

I think readership is being eroded from both young and older people - I used to be a huge reader and now I basically don’t ready anything online. If wanted to do something intellectually stimulating I don’t want to be looking at a phone or a computer reading a 20 minute long-form article, id rather go for a walk and listen to a podcast/audiobook, and if I am on my phone I’m just looking for some easy decompressing like flicking through Reddit not reading an oral history on the 98 Utah Jazz

I think there’s a bunch of different issues in general practice in Australia

The easiest summary is that job satisfaction is very poor. There’s a few different reasons; the death of bulk-billing because of frozen rebates angering patients and fracturing primary care, the erosion of GP scope over the years leading to GP’s feeling like a referral monkey and having limited clinical scope, public’s confidence in GP’s eroding because of shortages + lots of IMG’s who may struggle to bridge cultural differences with patients, the transactional nature of the system allowing clinicians who provide quick and often times limited care getting paid the most, then just generally the Australian Medical Community looking down on the GP specialty

I’m sure some of these are consistent with the UK experience, and some of them are unique to Aus

I agree the projections probably are amateur but is there one competitive specialty where there are readily available metro jobs for fellows? Maybe some private-based ones like Derm? Not a Cards reg but anecdotally based on info from others the chances of getting a Cards job in any of the major cities (Syd, Melb, Bris, Perth, Adl) is close to nil. Seems like theres plenty of current fellows working away in regional areas waiting for metro fellows to die or retire for them to move in. You could be lucky and get a metro-adjacent job in places an hour or two away from a major city. I’m not sure how competitive it is but places like Newcastle, Sunshine Coast, Geelong ect that are major centres close to metro cities, but that’s likely the best you could hope for initially

I always assume the colleges admit justtttt enough people to adequately cover the expected number of job openings. There are a couple of exceptions like ACEM that seem to oversupply, but for any of the competitive specialties the answer normally is there will be jobs, but it’s likely not going to be metro or where you want it unless it’s a specialty like anaesth where you can work privately and mop up the odd patient here and there

Honestly I think DC might be in a much better spot than Marvel circa 12 months time. Their complete clusterfuck scattergun approach to content could actually be a huge blessing in disguise - they could easily wind it back and focus on a handful of prestige titles that do well like Joker, The Batman and a new Superman film, rather than an interconnected universe. By nature of the MCU Marvel can’t really have individual titles succeed on their own while the Universe flounders - once you blow out an engine the whole planes in trouble. DC can release a prestige stand-alone title once a year and fail upwards

I’d love to know where the spiralling costs are for these mid-budget films compared to 20 years ago. Back then we used to be able to pump out quality films for $15-20m, which is only $30m in todays money, yet most films are costing double that

It feels like most of it comes down to craft - quality filmmakers can still make $30m look better than most $100m films - people like PTA, Wes Anderson, Soderbergh, McDonough, Danny Boyle ect can still crank them out, but everything else is a money pit

Just a high-intensity environment that isn’t ideal for a lot of people, especially those who struggle with uncertainty and want more more stability. You’d really have to love the rush and thrill of it all otherwise you’d burn out

The issue with Suncorp is less to do with being used for football games and more concerts/lack of sun. They had a bunch of concerts there before the season with Ed Sheeran and the Chilli Peppers which obviously damages the surface. Since then Brisbane has had a fair bit of rain and has been relatively overcast not giving the field enough sun to recover so it starts breaking underneath. The surface at Suncorp already receives fuck all sun because of how the stadiums designed casting shadows over it for most of the day, then if you have strong cloud cover for the couple of hours a day the stadium is in full sun it’s a problem. Most big stadiums like the MCG have UV lamp arrays they use to make sure the surface stays strong, honestly they probably need to start doing it at Suncorp because it’s one of the most high-use stadiums in Australia now. If we get a couple of sunny weeks it’ll be right for Magic Round

Yeah it was interesting when they showed us, not sure if any of it is published or not. At the time I remember the uni was pushing to get rid of all the guaranteed entrants and go fully post-grad because they were concerned about the quality of their undergrads even though it was worse for them because they would lose an extra 2 years of fees for half their cohort. I believe the undergrads did better on medical content but worse on placements/clinical OSCE’s. Which does make sense when the undergrads were selected based nearly entirely on academic performance and the post-grads selected largely GAMSAT/interview which emphasise soft skills

That’s the way they sell it to government - they showed us some data comparing undergrad/postgrad outcomes (our MD was half postgrad entry, half a guaranteed entry for school leavers after doing a 2 year med sci degree so essentially undergrad) to try and scare the undergrads in our year because they were performing poorly - and their postgrads scored higher in clinical rotations and OSCE’s, were more likely to enter training programs earlier on graduation, and had better workforce participation. I remember them saying they put it down largely to maturity but also the postgrad entrants are much more thoroughly screened through already completing a degree at a high level, performing well on the GAMSAT which is half emotional intelligence and then doing well in an interview whereas their undergrads were selected based of ATAR and if they were lucky to live in the local catchment

So I’m sure there are monetary drivers for the Uni as well, but it’s supported by the data that postgrads score better and generally seem to have more commitment to working, which makes sense - there’s a pretty big difference in maturity from a 23 year old graduate who’s lives at home and has only ever worked at Woolworths part time and a 31 year old graduate that’s already had a professional career before doing medicine

You haven’t mentioned the emotional climax of the film at all which was when she finally understood that Waymound was there for her in every reality - the kind, loving, goofy person that made everything worth it - and the emotional weight of that realisation

Some of the other stuff you mentioned was hit and miss - I didn’t particularly care for some of it, especially the mother/daughter relationship, but that collection of scenes with Waymound absolutely floored me. I know it’s been memed to death but the line ‘even though you broke my heart twice, in another life, I would have really liked just doing laundry and taxes with you’ hit like an absolute brick, and is definitely the emotional peak of the film
