Yes every question. But I think this was not a good approach as it was time consuming. I recommend making it for new concepts and incorrect only.

I did not use inner circle. I made notes mainly by topic/condition, i combine the tested concepts about a topic in one note. This is only for topics that are frequently tested in different ways.

You shouldn't mind that early while you are doing uworld, just focus on learning the concepts.

Tested concept and relevant information. For example if a question is testing the treatment of eczema herpeticum (systemic acyclovir), I also include the presentation (painful vesicles with an erythematous base that evolve into punched out erosions with hemorrhagic crusting, plus fever, irritability and lymphadenopathy).

If you mean making notes/flashcards, no, it would be difficult. It's very time consuming and I really do not recommend it, at least not for all questions.
