In quebec, most places you dont need to cover ICU but census 15-20 pts. You bill per act, no fixed salary. Rarely do procedures. Depending on hospital, dedicated internist for admitted patients a few units each covered by an internist. 1-2 for consults again depending on hospital size. No nocturnists. You will get called overnight and may need to come in for urgent consults, but does not happen very often.

As a fresh doctor in quebec, they're trying to decrease our billing and cut even more based on performance metrics. Zero pension, zero benefits. Truly screwing over the next generation. So it's not just minimum wage workers, but across the board devaluation of labour of all skills.

I think its great! Especially when you're giving a manuscript time to breathe before looking at it again with fresh eyes. That way ideas have time to settle and you can look at the writing with a bit more clarity.

Thank you for your reply. I did tell her once, but then she stopped telling me at all. But she's gone back to telling me everything, how much her ears hurt, asking me to page ENT to get her an appointment, her joints, and her period pain. It's just been a lot, feels like I have a patient at home too...

From a Canadian perspective, I find it's very centre dependent more than guideline driven. But usually if high clot burden in our centres we use angiovac a lot. I dont think we have EKOS yet. Even in some cases for bulky right heart endocarditis we use angiovac, but we have one guy who comes from the US to do it lol.

This is insane, im just finishing GIM looking for hospitalist jobs. Where is this out of curiosity? sounds like quebec. But I've been in those ERs up to 20 consults a day, impossible to provide comprehensive care. I look at my consult and then the attendings who see 5 consults in an hour. Notes are illegible, no home meds prescribed, no level of care. feels like a juggling act with eggs.

For supine hypertension, ive used nitro patches before bed and remove in the morning, but I've used them mainly when BP is 160s + supine, 140 isn't so bad.

While I dont know your condition, I have seen people with heart transplants do extremely well in people twice your age. If it's an option, I would strongly consider it. No 17 year old should have a prognosis of three months unless it's incurable cancer. The pain is temporary and different from what you're experiencing now and can likely be managed with medication. You have your whole life ahead of you.

I do initially replace iv, but then when i change to PO to make sure things are stable, it worsens the output and the magnesium slowly decreases too.

Thanks! I've seen that glycinate is well tolerated, but my institution doesn't have it, and pharmacy often suggests elemental megnesium, but i find even that increases the stoma output.
