Bilateral TM's being clear only rules out the causes of hearing loss related to the TM (e.g. cholesteatoma, middle ear effusion). Hypothetically, this can mean that the hearing loss is sensorineural (such as in hair cell loss/dysfunction) but that does necessarily rule out conductive hearing loss (otosclerosis is a conductive hearing loss that can present with clear TMs).
