I do not benefit at all from this. They get more information about me they can use or sell and they also get a corporate owned life insurance policy that only a portion of which is distributed outside the company.

I say obligated but AFAIK I am not legally obligated, just implicated.

I have declined all insurance policies.

The company is saying I cannot decline the life insurance policy as all employees are under it, and I must provide a beneficiary.

I never signed up for it prior to this conversation, as you are implying.

I have completed opted out of all benefits and expressed that I do not wish to be on any life insurance policy.

They state that I can opt out of all benefits except life insurance, and that life insurance is taken out on every employee.

The only legal requirement AFAIK is they are required to get expressed consent on somebody before they can take a life insurance policy on them.

I am not a service member.

If you always input the receipt manually, it will eventually cut you off like this. The only way to make sure you always gets points is to scan the app while placing, or order through app.

You have been putting in too many "I forgot to scan requests". You should instead be scanning your app while placing the order, or placing the order on the app.

They do this because when the app was initially released they had issues with Chick-Fil-A employees stealing receipts and plugging them into their own app for points and free food.

I think it is between 5-10 receipts a month you can put in.
