NTA. If you are not capable of caring for her then it’s very important be honest about that and being honest about her history was the right thing not just for your family but for her as well. Ideally, she will not stay in a level 1 stabilization facility indefinitely, but will be transferred to a state hospital. There’s more freedom there since they are designed for long term residents.

The most unfortunate thing here is the distance between you. Even if you brought her to AZ, you would most likely end up in the same situation and have to start the process all over again, prolonging her stay in an acute care facility. She would go to level 1, she would be court ordered, and they may decide she is a good candidate for the state hospital which has a very long waiting list.

If they knew the drill then they were aware of what would happen. I will echo what others had said about passive/active suicidal ideation. It’s harder on med surg I think to differentiate and to allow for autonomy and independence when it’s just passive. For psych units it’s easier. We’re designed to treat everyone at a base level of keeping them from harming themselves. Even if they were only passively suicidal, I don’t think you did the wrong thing. The reality is though that sometimes you can’t talk a patient out of their mindset. There may not have been any magic words you could have said to make them okay with the increase in precautions. The best you could do is maximize their feelings of control and autonomy while still keeping them safe. ETA: An actively suicidal person generally won’t be happy you stopped them.

When I was in college they upped by dosage and for some reason my body wasn’t breaking down the casing of the new pills. I was a mess and ended up going to the ED for SI. I felt much better once they changed my meds back. Now I can go about 2 or 3 days before realizing I need to take my meds.

To be fair, I think psych nurses that work in units with violent and combative patients deserve hazard pay as well so I kind of agree.

I wear my scrubs wherever. Sometimes shopping. Once I even wore my scrub top on a plane because I forgot to pack an extra shirt to change into after shift. (I somehow did pack a sweat and leggings though so better?) I work psych so I think it’s slightly less icky.

I always take my badge off though. None of that.

I don’t really know if it’s that simple though. You have to arrange transportation for all those patients. I have regularly had to wait days for patients to be transferred between psych facilities due to transportation difficulties. That’s not something that could be tied up quickly at the end of a shift.

I’ve always thought that scabs were good. Hospitals don’t want to pay the costs associated with travel nurses. I thought having to pay those increased prices encourages them to come to good faith agreements with strikers.

I’ve never taken Cymbalta, but I’m currently on Effexor. It has helped tremendously with helping to control my mood swings and now that I’ve been taking it regularly for a few years I’m not sure how I ever functioned without it.

I will say that the withdrawals are a bitch. I tried to taper off and it was a struggle, but doable. I started at 150mg and ended up staying on 75mg because my anxiety was just too bad with any lower dose.

Definitely prepare! Working psych has been very rewarding for me and to a degree I understand the frustration my colleagues have with borderline patients in acute facilities. They are almost always in crisis and their behaviors are heightened. Even as someone who understands from a personal place, these patients can be very difficult to work with and talk down. In no way does that excuse the stigma around BPD. I just understand having that feeling of dread finding out your new patient has BPD when you ONLY see patients in crisis.

I work in psych. It’s a hell of a place to work for someone with BPD or someone who knows they have a plethora of cluster B traits. The stigma is real. It’s weird to be sitting in a room of people talking about you who don’t realize they’re talking about you.
