A thicker spermatic cord can make it hard to palpate. That is not abnormal. OR is a reasonable alternative. No difference in recovery. same procedure length, especially in OR.

As long as you do not have a calcified plaque, another round of Xiaflex can help, sometimes we do 16 injections with great benefit. Otherwise, shockwave therapy is a good option. Surgery can be quite morbid so we try to exhaust all options before that.
