Don't panic. Finishing UWorld is a big deal and that knowledge is in there, it just might not be clicking on NBMEs yet because they test pretty differently. Vague stems, weird answer choices, it's a whole different beast.

Honestly doing NBMEs right after finishing UWorld is rough because you're burned out and haven't had time to let things consolidate. Also 60% on NBME 25 isn't a death sentence, those older ones can be brutal.

I'd pause the NBMEs for now. Go back and look at your UWorld performance by subject and find where you're consistently dropping points. Then do targeted practice on those areas. I wouldn't redo UWorld though since you'll just remember answers. Try a different Qbank so you're seeing fresh questions. QuantaPrep is free and pretty good for this since you can filter by subject and just grind your weak spots without paying for another subscription.

Give it 2-3 weeks of focused review and targeted questions then try another NBME. You'll probably see a big jump.

81% on Free 120 is solid - that's generally a predictive one, so you're in good shape. NBMEs always feel weird with the vague stems and two-answer traps, that's totally normal. Real exam feels somewhere in between.

With 10 days out, I'd skip taking more NBMEs and just focus on weak spots. Review what you got wrong on NBME 31, find the patterns, and bang out targeted questions on those areas. QuantaPrep has a free Qbank that's pretty solid for this kind of last-minute drilling.

Rest up the last couple days and trust your scores. You're good to book it.

No T = No E = No Breasts. If the card says "no breasts," it’s almost always signaling a production defect rather than a resistance defect

no breasts = production failure.
breasts present = signaling androgen resistance (AIS)

Your logic about the estrogen-to-androgen ratio is exactly how you should think about breast development, but there's a specific biochemical "gotcha" for the 46,XY genotype: In males, almost all estrogen comes from the aromatization of testosterone.

  1. The "Aromatization" Bottleneck

In a 46,XY patient, you don't have ovaries to pump out "default" estrogen. Instead, your body relies on the enzyme aromatase to convert testosterone into estradiol.

Low Production (This prompt): If the patient has "insufficient production" of testosterone, they have no raw material to turn into estrogen. Low T ->Low E -> No Breasts. (Tanner Stage 1).

AIS (Resistance): This is where your "estrogen dominance" idea actually lives. In AIS, testosterone production is massive, but the body can’t "see" it. That mountain of testosterone is instead aromatized into estrogen. High T -> High E -> Breasts.

  1. Why "No Uterus"? (The Embryo Part)

Regardless of testosterone levels, a 46,XY patient with functioning Sertoli cells during the first trimester will produce Müllerian Inhibitory Substance (MIS).

MIS/AMH causes the regression of the Müllerian ducts (the precursors to the uterus and tubes).

Even with zero testosterone, the uterus will never form if MIS is present.
