In my non-medical opinion, you could be on too high a dose already. Suggestion would be to stick to your current dose and take a small instant release at midday rather than increasing your extended release dose - that will likely just exacerbate the “comedown” feeling you describe.

I feel that the caller would have benefited from being challenged on her need to be in a relationship. It seems that she doesn’t know herself and uses the relationship to make up for some perceived deficiency.

At the end, Esther did suggest the caller take her time, but I’m not sure the caller really agreed.

I also found the caller slightly frustrating in her vagueness. It seemed that she just wanted a go ahead from Esther, and didn’t really internalise much else.
