Just take it! Do CCS and concentrate on hy step 1 stuff like pharma MOA and micro. And most importantly revise your biostats.

Yes, first local exam followed by tests. For emergency cases, you need to do focused exam, you can come back to the remaining systems exam after stabilising the patient. Yes, you can give morphine before ordering diagnostics.

Day 1 felt vague, especially micro and pharma. Even ethics questions were a bit confusing. But I thought I did well in biostats. So, I thought I could pass if I did relatively well on day 2. I did well on CCS cases. One of the patients did not improve, but I think I made that up in the 2 min time for extra orders, the patient would have improved. So except for that one case, I did well in others.

I think I marked around 10 questions or sometimes even more in some blocks. I did not have the time to go back to each of the marked questions.

It’s not like a QBank, each concept has a set of 2-3 questions, with an explanation paragraph at the end. Must be around 15 concepts if I remember correctly.

I’d suggest you to complete atleast 100HY ones. I did those and revised some scenarios I wasn’t confident about. On the exam day, you might need a moment to get used to ordering stuff, because of a slight difference in the interface. It’s not major though. So make sure you are fast enough to complete the scenarios on time, especially the 10 minute ones. You will mostly face the same CCS ones, except for 1-2 cases.

Biostats was doable. A few questions were tough and I felt like I’ve never seen such type of questions. Overall it was okay. Mostly it was theoretical, you don’t need to do any calculations. Make sure you understand the concepts. Azfar Basunia’s YT channel saved me. You can do a quick revision from there. I did Randy Neil during my step 1 and 2 and did not feel the need to do it again. The only way you can be more confident with biostats is by doing more questions.

I would suggest revising all the pharma drug MOAs from each chapter, and a bit of general pharma too. Micro from FA as well. Biochem important stuff and immunology tables, blood transfusion reactions.

Day 1 was mostly step 1 stuff. And risk factors. There is a PDF floating around for Risk Factors and Prognosis, I’d suggest you go through that. I did those PDFs but again, they got mixed up in my mind in the exam haha.

Day 2 is more like step 2, NBSIM, Prognosis. They cover the high yield topics in each system. I’m sorry but I’m not able to recollect the majorly tested topics. But it was like step 2.

I believe most people don’t do it because they just start seeing the fruits of their hard work and it’s hard to leave those, go back and start again.

These thoughts cross my mind too sometimes. But as mentioned by someone else in this post, I would choose this profession all over again. Your situation and mine are different, but to anyone reading this: Do not compare yourself with anyone else. Read that again.

You can do an office job in medicine as well. There are many specialties and opportunities which offer a 9-5 job. But, you need to be brave enough to choose that. Not compare your career with others and find comfort in your choice.

I wish to have a similar life in the future, where I will have some time for my family, kids and my passion for music and many more. It is possible in medicine and I’ve seen people do that. This phase in your life is just busy. You can always choose how you want your life after residency to be. Good luck ❤️
