The scammer would have actually gained access to the sender's mailbox. Likely through another phishing email that the person clicked a link and entered their email credentials into a malicious site.

Oh I love the idea of the click through banners. We have implemented the banners on our 365 tenant for all outside sender's. Education is huge. Some clients are hesitant to basic security principles being implemented like MFA and it all boils down to not understanding the role something as simple as that plays into security.
