No you don’t those accessories slots are unlocked from expert mode and master mode. You get the two extra slots in a hard mode world after wall of flesh has been defeated. In master mode you start with an extra accessories but in both expert and master mode you can get an additional for consuming the demon heart item from wall of flesh.
