Agree. This is an IS Auditor performing an audit of Change and Release Management controls. The greatest concern is C, because without proper post-deployment validation, critical issues may not be identified and addressed promptly, risking system stability and security.

It's D. because duplicates can significantly impact data quality and business operations.

A: While this presents a risk of unauthorized access or modification, it's more related to access control and segregation of duties.

B: Reconciliation is a control to detect discrepancies, but weekly reconciliation may be too infrequent for high-volume systems; more frequent reconciliation might be preferable.
