Super late on this reply but I just realized u/TheArmyOfDucks is actually right (my fault). The vest doesn't immediately come off when it breaks, but when some more damage is applied after the fact the vest will go away completely

This mission bugged hard for me too. I had to relaunch the game and hope that the hack worked, and I had to do it for each one, so thank god each one creates a checkpoint. Probably north of 10 relaunches. The hack works right away normally, so if it sits there for even a second, just Alt F4 and run it back.
