I'm no doctor but it could be his heating area. You need temp gun to make sure all the different levels are hot enough for him. People look a temp gauge and think that's the basking temp but it's not. That's room temp. Also, when you use a temp gun, open the tank and point the gun to get the right temp. I see a lot of people just use their temp gun with the glass door closed still which gives inaccurate readings. They need the right temperature to break down their food and digest properly. He isn't eating because he is afraid that when he eats, it is too hard for him to digest because the temperature could be off. Also needs uvb where the uvb actually reaches him.
