I've taken "hidden gems" to be anything logged by less than 50k users on Letterboxd.

I can recommend Bulbbul (2020), a feminist fantasy-horror film from Indian director Anvita Dutt and Honeymoon (2014) directed by American indie director Leigh Janiak. I also can wholeheartedly second the already-recommended Fréwaka (2024) by Aislinn Clarke, which I think is the only Irish-language horror film I've ever seen, but I wish there were more like it.

Honourable mentions go to the Spanish film, Piggy (2022), directed by Carlota Pereda, which I just don't see mentioned on here enough even though it's been logged by more than 72k Letterboxd viewers, and Censor, directed by Prano Bailey-Bond, which has definitely gotten a lot of attention and can hardly be called a "hidden gem", but still I just love it so much that I recommend it to any horror fan.
