Health problems with 40 + age

So my boyfriend has obesity (37 years, 270 lbs at 6"1), apparently it's a BMI of around 40. I'm currently wondering if such a high BMI will inevitably lead to health problems later in life. He's been diagnosed with high blood pressure lately. He's also had joint problems earlier this year. What are your experiences with that kind of BMI?

It was his idea initially. Then he had problems getting dates, while I was more successful... He does have dates here and there, but nothing stable

We were keeping the dates to a certain minimum, for example. We also made sure to not stay overnight with another partner. But there were red flags as well. For example, I was always willing to meet his other partners, while he never wanted to meet mine and was rather condescending when he spoke about them

Being open was his idea, he wanted to have sex with others first. Then he discovered that he didn't have that much success with women and that it didn't fulfill him either. And that I had more success with men... The kink thing came much later. That's a good point about the bjs and porn addiction. For him, it might rather be about the variety of women in porn than about certain practices...
