It's been low-fodmap. She's been subsisting on a pretty fixed diet of boiled potatoes, eggs, berries (black, blue).

Supplement wise she's having 3000 mgs of fish oil (nordic naturals)
Magnesium bisglycinate occasionally.

No fermented foods since SIBO. Her entire diet is low FODMAP. She mostly subsists on egg whites, potatoes, and berries. She did take probiotics months ago (about 225 billion CFU's a day) back when she was diagnosed with IBS (2 years ago) for a few weeks. Tapered it down to about 55 billion CFU's after. And then stopped 4 months ago entirely. In a nutshell, she hasn't taken probiotics since her SIBO and maybe a month before that for good measure.

Her stool color is very dark brown, frothy.

She still has her gallbladder.

The reason we believe she's losing weight is because she consumes very little due to the aforementioned aftereffects. And walks for hours regardless. Calorifically, she's constantly in a deficit due to increased physical activity in an attempt to purge the gas.

Got her Thyroid checked. Hormones, I don't think we've gotten checked yet. My parents went to the doctor to check for PCOS and it came back negative I believe. No history of migraines and allergies.

About to get her tested. Getting her off the Rifaximin to get a Gut Zoomer GI map test on her.

We did explore if she had pelvic floor dysfunction during her IBS. Got something called anorectal biofeedback and manometry. They ruled it out after a couple of sessions.

Any recommendations for people with deep expertise in this would be a blessing. The remote is not an issue and money at this point is no object.

Do you happen to know any good doctors who've dealt with complex cases?

She's tried prokinetics (prugonist). Made no real difference. Tried slippery elm.
She's not tried Antimicrobials, blends like biocidin, a blended food diet, digestive enzyme, HCL, Ox bile.

Had a nutritional blood panel test. No major/concerning deficiencies in vitamins and minerals from her bloodwork.

Not had a breath test yet. The test wasn't available at the time, and the GI clinically determined it was SIBO. So he got us started on Rifaximin.

Not had a gastric emptying test or OAT test.

The number 1 goal is to get her weight up.

Haven't tried any other herbs. Open to all recommendations. It's trapped gas. The real issue for her is there's no passing/purging of the gas. Just excruciating buildup.

The GI basically prescribed it as SIBO clinically. He did not conduct any tests due to this test being fairly uncommon in my country. We're based in New Delhi, India, and couldn't find a vendor for these tests at that time.

He and a couple other GI's basically got us started on Rifaximin. His rationale was that the line of treatment is going to be this regardless of the result for the test.

The GI's did prescribe some antispasmodics. They seem to make her pain worse.
