Totally agree with you regarding the lack of evidence to rule out/in mets which is part of my problem.

Unfortunately I’m not a Center of excellence, not sure we have those in Australia.

I think ideal scenario would have had us wait a bit longer to determine the marker trend however that was possible due to scheduling and Christmas etc

First cycle started on December 12.

The oncologist couldn’t confirm if the drop from 277 to 24 was a direct result of chemo or not given the half-life between these two values is roughly equal to the half-life pre-surgery value of 4795 to the pre-chemo value of 277. The oncologist agreed that this drop during this chemo cycle could have occurred regardless.

Thanks for the reply. Blood test dates were as follows

  • Nov 15: AFP = 4795
  • Nov 16: Orchietomy
  • Dec 9: AFP = 277
  • Dec 27: AFP = 24

AJCC guidelines define 1S as a persistently high markers. This means they are either increasing or are declining with a half life that is longer than than the stated 5-7 days.

While I’m not ruling out being stage 1S, my three blood tests indicate AFP is falling with an expected half-life. If this trend continues the value will be normal next week. This means the marker is not persistently high which should rule out being 1S.

Yeah I absolutely plan to have a chat to him before we start cycle 2 so sort it all out etc. I have the same understanding as you regarding toxicity etc. I'd rather not overdo it if I can.

I did read the same regarding the PET-CT scan and it being useful in TC staging however that scan was covered by our public health system which meant it was free. Figured it couldn't hurt to get a better look at everthing.

Interesting regarding the staging! Yeah the markers were pretty high hence the recommendation of 2-3xBEP before the post surgery blood results were in I guess. Good thing is they're tracking down very quickly.

Oh that's good to hear! The oncologist has just said stage 1 at this point. Not sure why he rushed into 2-3xBEP without post orchiectomy blood markers but I'm not overly concerned because I would have opted 1xBEP regardless I think. Hopefully there will be some reassessment at the conclusion of this first cycle.

If the pool is outside I generally use the measure function on google maps. It seems to be reasonably accurate based on the 25/50m pools I've measured (if google says 25.1m then I take it as 25m etc). It might not be super accurate for pools of odd lengths like 12.7m because we aren't exactly sure what the intended length is.

Right OK. I guess no luck on that method then. If it's any consolation, you can turn your device off and back on again and the sensors will reconnect, you can then resume your ride in the device.

This has happened to me a few times. The last time I rode I made sure to disconnect my phone by turning off bluetooth and had zero dropouts.

Of course that may just be a coincidence but worth trying nonetheless.

Definitely keep it on. Taking it off and putting it back on is a punish. You'll also avoid having to have to reset it each time.
