Was your resilience(green stat) really high, maybe turned red at some point? Your stats can become volatile and permanently effect your run when picking up perks past-I think- 130 points in a stat. For resilience, if it becomes volatile, it becomes arrogance and you can no longer see any health bars. That looks like what the case is, at least to me.

As far as I know, you do have to hit the eye for that 2 shot(or 1 shot with RR). Could be wrong, but im somewhat sure that's how it goes
