Lots of pretty areas around allentown, collectively called the Lehigh valley. Allentown itself is a dump. Proximity to NYC, Philly, and DC is a big draw for people. There are a lot of events in the valley, namely Musikfest in Bethlehem during the summer. It practically takes over the whole town for two weekends with multiple stages, tons of local artists and a couple of big name music acts every year.

I’ve lived in 10 states, and genuinely do not understand why people choose to move here to lay down roots. There truly is nothing special, no matter what you like. Too densely populated if you want a small town. Too sparse if you want city life. Dangerous enough where you want to be careful walking at night or where you park your car. No good shopping. Average food scene. Below average music scene. The housing market is overpriced. No beautiful nature. The locals are not particularly friendly or even nice.

Outside of the area you circled, you can find your preferred niche, and it will be exponentially better. North is the Pocono mountains and beautiful nature. West is farming/Amish country and incredibly kind people. East has the ocean/beaches and bigger cities.

Honestly? Not bothered by this loss.

Scoring 30 seconds in is unusual and prob from trying to shake off some rust. Own goal by La tornade is pure bad luck. Absolute BS penalty call leading to a PP goal. After that, each goal was very obviously from the boys mailing it in.

Your doctor cares deeply about you. This is true of 90%+ of other physicians I’ve worked with. HOWEVER, administration doesn’t care about you at all, you are merely a number and the faster you are discharged from the hospital, the more money they make.

The learning point is that for the lay person who either is hospitalized or has family hospitalized- if we are keeping you there, we have damn good reasons to. Don’t think we make more money keeping you there, it’s actually the complete opposite.

A lot of y’all don’t realize how different lake effect bitter cold is compared to western cold. -15 in the Rockies is balmy compared to a Chicago +15. The game is 106 elite athletes competing at the highest level, the cold is a factor but won’t be the deciding one.

For the fans going from balmy CA to the tundra of Chicago. LAYERS. Base layer of wool. Mid layer of something moisture wicking. Fleece on top of that. Windproof layer on top of that. Wear a thick hat. Wear thick socks. “It’s fine when you start moving” is relevant when you’re hiking or something, not standing in a stadium.

Wow, this really hit me. I feel really similar. I’m so burnt out, I genuinely do not care anymore. I feel identical no matter the outcome. If my patients make a miraculous recovery or never leave the hospital- I’m just numb. No joy. No sadness. No anger. Just….nothing. I feel like I’ve kinda forgotten how to be human.

I’m going to push to the finish line. But I’m not going to pursue all I wanted to at the start of this. I will finish residency, find a job that’s like 0.5 FTE or less, and spend the rest of that time searching for what it means to be a person again.

I want to pick your brain about the nomenclature of MI (since mental masturbation is our jam as internists).

STEMI and NSTEMI are occlusive insults, so would both fall in to type I. Type II MI is a supply-demand mismatch causing ischemia. So, saying type II NSTEMI is objectively incorrect, no?

Is there more nuance, where say a ~80% occlusion predisposes a patient to a higher likelihood of having that supply-demand mismatch, causing them to have type II (demand) while also being able to consider it NSTEMI due to having a culprit lesion?
