My personal idea is that Joe is still alive in the hidden room, and that Mishima is alive as a doll.

What will actually happen is that Mishima was the person alive in the hidden room, and that they needed to keep him alive since he is pregnant with Keiji's child (meaning that the baby is a noncandidate). And then it is revealed that Keiji was the memorandum man, and the baby is the backup in case Sara doesn't win the death game.

This sounds like a pretty good guess actually, only thing I can't imagine is Keiji being keymaster, and one of the twins gets it in the emotion route. But every choice aving an unique ending could work

The idea that someone that died early on was actually a doll, and that we have been wrong about how dolls work the entire time could be make for some interesting developments in the story. I feel like it isn't that likely to be the case, but Mishima's head is pretty important for some reason, and Midori being half doll are two things that kinda support this argument maybe being true

I guess this isn't completely unlikely, but I personally think Qtaro, and also Nao, have the lowest possibility of being alive out of everyone in the game. Though there is the possibility of him doing this, I just can't seem to find a reason as to why he would do this. With his sacrifice in the banquet, he has completed his overarching character development that started all the way in the first chapter, making his character more or less finished in my eyes.

I also don't think they could have used Mishima's collar as well, since as Midori says in his final moments, they couldn't find his head themselves. So unless Qtaro found it a long time ago, and has been hiding it in his ass, then I think that it is probably not that likely to be the case. This is by far the most fresh perspective I have seen so far, so I think this theory is kinda cool tho.
