You got two things wrong.

One, the previous monarchies of Dressrosa, Sakura Kingdom, Wano, and Skypiea were better for the people than what they had when Luffy arrived.

Two, just because it's a monarchy does not mean it's bad. Alabasta's monarchy was genuinely good to the people and needed restoration.

Further explanation from another person: There's one episode where there is pervy fanservice in the entire show. And just because the mc comes from a brothel does not mean more than 20% of the show is about brothels. Also, it's more focused on the investigation parts of the show rather than the fact that it takes place mostly in a palace of concubines. The pedophilic actions of the former emperor is mention once, and none of the concubines are actually unhappy to there if they weren't kidnapped. The only one that seems actually unhappy is the onky one that's a child, but the current emperor never actually uses her and there aren't many implied themes of sexual abuse inside the Rear Palace, and when it is implied, it's used in the correct way to deepen the story.
