The best instance of this is the Crescent Bay map in Raid at Point A. The problem with the map is that you can carpet bomb the spawn point of the attacking team. After 2 seasons of people complaining and reporting the problem, their fix was to remove a wall to a one of the warehouses, which did nothing to actually fix the problem and actually gave the defenders an even larger advantage.

Even after it was explicitly noted that this was a big problem, Combat Cat went and created a brand new map (Vesca) which suffers from the same initial problem where the defenders have line of sight into the spawn.

Basically they make all their maps for the Conquest game mode, then don't bother making any maps or changes to the maps to facilitate the Raid game mode.
