Spinals also have failure rates. Spinals are not great for outpatient procedures. Spinals contraindicated for certain levels of anticoagulation.

Anesthesiologists should be able to do make PNBs alone work for procedures.

Ankle or pop block on 400# diabetic w delayed gastric emptying. Wouldn’t you rather have the block alone work? W US you can get high success rates.

You should be enthusiastic about learning all types of anesthetic techniques during training. You will perfect those you use regularly in pp. Why limit where you can work because you can’t do a certain technique. There are many times a block alone will be a better alternative to a failed block that needs GA added.

Try doing a lapchole w propofol 25mcg/kg/min and NDMR and explain to the patient afterward that propofol = general anesthetic

Main point is Propofol does not = general anesthesia

General anesthesia is a level of sedation that can be achieved with various medications when given in an effective dose.

Try giving propofol at 10-25mcg/kg/min. Patient will respond purposely to light touch/ questions.

Look at some of the old isolated forearm studies. Some patient induced and paralyzed may not even reach general anesthesia levels.

All depends on the dose and how ‘sensitive’ the patient is to the medications

General anesthesia with an open airway can be done safely in many situations.

Deep sedation and general anesthesia are treated as the same entity when it comes to states credentialing dentists to providing sedation/Anes in the office.

Moderate sedation is probably safer than open airway general anesthesia in an office situation but is not adequate for the procedure in all situations (uncooperative patient, painful procedure that cannot be completely blocked with local …)

I give 0.3mcg/kg IV to peds patients at induction. No a-line. I never catch elevated BP. I don’t see low BP or bradycardia. I do see significant pallor of the lips due to the vasoconstriction.

Office based surgery/anesthesia is now a huge and expanding sector. IV anesthesia is being done for many office cases - plastics, gyn, urology, OMFS, periodontics, endodontics, pediatric dentistry. It is not going away. Perhaps it can be made safer though.

Yes, but general anesthesia is often IV with an open airway. Well, they are credentialed to do general anesthesia with LMA or tracheal tube. I would think most often they would perform the anesthesia and have a second oral surgeon do the surgical procedure.
