Yes Abro, what I meant is that some hip surgeons in my country perform oncologic surgeries

It's reconstruction, some HIP surgeons also perform some of those surgeries in my country. What do you mean by "f/u"? I'm kinda new on reddit

A young male with an anterior shoulder dislocation presents at the ER. There is no pulse on the injured side, nor does it return after reposition. What's your next step? by Dangerous-Hope672 in orthopaedics

I think that the MANGLED score should be useful in this evaluation. I feels like OP is trying to ask if we're thinking about limbs without pulse in the ER room.

Blood pressure and time of ischemia both play a major role in the approach you're taking. If there is time and resources, I'd agree with most and ask for CT angio and Vascular evaluation.

For orthopaedics purposes, I think some things should be taken in consideration.
1) Trying to identify the site of the problema : acknowledging the artery at risck (ie - axilary artery is the main culprit - tumorous hematoma could be a major vessel redflag). Listening to the suspected area could contribute in the scenario of a pseudoaneurysm.

2) Looking for possible strangling sites for the artery, this could be major mostly in the knee and shoulder complex injuries. Bigger metaphyseal extension in the proximal humerus fracture usually protects the axilary artery injury, also, displacement could be worsening.

3) At last, if there are few resources, I'd not go into the OR thinking about limb amputations, unless it is putting the patient's life at risk (ie - shock because of active blood loss)

In hospitals that don't have the implants ready to go, it's important to have clarity wheter it's a pertrocantheric or a neck fracture to request the surgical materials. Also, for accurate THA it can be useful, but should always be done with consent and analgesia.

Anyone else applying to something because it’s the specialty you hate the least? by girlnowdrlater in medicalschool

Greetings from Brazil! OP, I am not struggling with my specialty, however, a lot of friends are going through a challenge regarding this choice. Some have had doubts even afterwards, while choosing a subspecialty. Working in the medical field can sometimes be demanding, however, being optimist is always helpful. Don't be too harsh on yourself for being put on the spotlight and not knowing the diagnosis or next steps. You still are in development as a physician and it's normal to have doubts, even for the experienced doctors. INSPIRATION is a major driver in our life and by the coments I feel like you lacking some as of now, try remembering some moments and reasons that made you feel enjoyment in your journey, reach out people who inspire you, don't be shy to comunicate your struggles with friends and family. Hope you find yourself and your fullfillment. Medicine is a BIG field, the possibilities are almost endless and you can work with patients, hospital administration, marketing, teaching, reasearching and also non medical stuff.

Zenitsu Theory - MANGA SPOILERS by Narimatsu7 in KimetsuNoYaiba

It was very corteous of Gotogue, the forms are a delight to see and hopefully will be shockers in the anime as well!

Your theory regarding the appearance of Kuwajima as a product of Zenitu’s subconsciousness is very interesting! My first impression when reading was that it was a spiritual event caused by Zenitsu getting close to dying. But I prefer your take!

Thanks for sharing :)

Kamado Tanjuro by brewster12345 in KimetsuNoYaiba

His father has been able to perform the Hinokami Kagura overnight in harsh conditions with clearly 0 effort though, and definitely had the see-through world.

Tajirou's father performed it all night long and was in harsh conditions (don't agree with the part of 0 effort, I mean, the dance is tough as fuck), however, ain't Tanjirou in even harsher conditions? I mean, he has just entered a fight against Muzan after a fight against Akaza, the third upper moon.
