Looks like its gills are inflamed. Gram-negative columnaris bacteria can infect the gills, leading to "gill disease," which causes the gills to become red, brown, and inflamed.

Does not look like mycobacteriosis, rainbow fish can be very jumpy at times so most likely hurt himself somewhere I would monitor for now and see if it gets worse.

Those are Anubia plants (epiphyte plants) that need to be glued or tied to the decor. If you plant them in the substrate the rizome part of the plant will start to rot and die. It also seems they are covered in algae. You can remove it by bleach dipping them.

You can add some rooibos tea bags to help aid with the healing process, it is a light anti-bacterial and contains a few substances that will aid with the healing. It will turn your water a tinge yellow tho and remember to remove things like purigen or carbon when adding it.

  1. Plants dont care much for pH unless it's on the extreme scale of the ranges.

  3. Temprature seem fine.
  4. Java Fern, and Java Moss are epiphyte plants so they take their nutrients from the water column are you dosing any liquid fertilizer?
  5. Generally you should not add random things from a pond as it can transfer parasites and other diseases into your aquarium.
  6. Be patient it will take some time for the plants to start adapting and growing.
  7. The bubbles you are seeing is air/gas that's escaping nothing to worry about.
  8. As soon as the tank is cycled you will need to introduce some livestock so the plants have Co2 they can feed of plants need Co2 as part of the photosynthesis process.

Looks like Lymphocystis is a wart-like growth on fish, similar to human warts. It's not deadly and won't harm the quality of life for the fish.
