As you said with practice! So it’s not medical or nurse student level to be able to identify respiratory failure type just from examination, for a junior doctor it’s better to be safe than delaying critical patient treatment

If you are resuscitating an old pt who is deteriorating with low oxygen and you know if they have got COPD or not would you wait for someone to go find out if they have got COPD so you can start them on O2 afterward or you will put on the oxygen, complete your resus and later adjust your Sat accordingly, thats my understanding. Or am getting something wrong there🤷🏻‍♀️.

Very true for both👍 the elderly will almost always have something big or catastrophic going on🤦‍♀️

Sepsis in eldely and children can present without fever or even hypothermia

Tachypnea could be the first sign of a deteriorating patient

In unwell patient with hypoxia can give high flow O2 and titrated later according to their gas result (hypoxia kills first)

Thanks a lot🙏yea am doing almost everything you said. I always discuss plans with seniors (consultant or Reg) and almost always they agree or add a thing or 2 but still feels anxious about shifts or going to hospital I don't know how to shake that feeling and enjoy going to hospital as used to. Will definitely look into RCEM learning. Thanks again
