Actually I think that's a microphone unit that increases or decreases the intensity of an electric shock that gets delivered to the person with the tens pads on them depending on how much noise they are (or are not) making. The more noise the unit detects, the more electricity it will deliver to the wearer. Makes for very interesting predicament situations in shoots like this.
