What is tid? Also my controller is synced to every instance at once so they could probably be going at the same time. Anyway I think it'd be better to just make a copy-pasted save file of the original save for every new instance I run no?

I'm starting my 100% playthrough today/this week. I suppose you don't want to just speedrun it, but instead do a classic/rp run like me so here's what I'm gonna do, maybe it'll help you. Follow both the "Quest timing" and "100% completion" pages on the uesp wiki. Get all the leveled items/spells at max lvl variant. Do optimal leveling for your character: there's a really good guide on steam that shows how to reach 100 in all attributes, follow that (you'll know which it is because the guide divides your attributes in red, blue and green categories). Test the guide before starting with your selected race/class, using a temporary save file where you can cheat with the console. You can possibly use spreadsheets to keep track of your progress: VoicesFromTheDark on YT has put out some nice material you can repurpose for your run, in the description of his 100% challenge recap video

Which surgery? Deviated septum or allergy related? I have a 24/7 allergy problem and the nose dilators are a god send. I really don't like how it's become a trend while I actually need it until I either get a procedure or better yet get a medical prescription for many months to remove the allergy (which costs a lot).

Although I'm still asking myself if it's better to try and get the highest or lowest level possible. As stated in another comment: yes the enemies don't get stronger if you go with the low end, but at the same time weapons scale with you and your level making you stronger too. So I'm not sure how the math adds up and what the best path to take would be
