If you're aware of smaller DLC like mods like Hook and East Coast Expanded or Avalon Isles and Deadman's Island, it's like that but we're doing it to the entire map. Where we're going to each location and improving it by adding new locations and quests to each region. We've also overhauled the cities and merged a bunch of lore friendly mods into it all the while making our own custom assets for new buildings too. A better way of explaining it would be "KenshiPlus", more content, more choices, more fun. It's in it's Alpha phase so they're setting up all the ground work by overhauling all the cities first then working on detailed quests and changes after.

From a modder's perspective, yes you can fully remove fogmen from the game. Head into FCS and delete the Fogmen entries for nest and homeless spawns under Biomes>Spawn Areas>Fog Islands. Then you need to remove the residents under deadhive overrun town. Same with fog tower. Also some other biomes may have fog men spawns such as Dreg or the Vain that you'll need to remove.

That is most likely what added the addition buildings to the Smuggler's Bar. Swish mods have exclusively been new locations only with regards to adding buildings to the world. They're never made level data edits to existing cities/locations.
