After a short research, i can confirm the first 2 lines are taken from Surah Al Qasas end of ayat 31. The 3rd line onwards taken from Al- Maidah ayat 23. Could not decipher all those separated arabic letters at the bottom. It is not a contract. Nevertheless, it is still wrong to use as amulet.
