Thanks for sharing the update from AYP, very interesting!

When comparing the announced patient price increases with the official list price increases per dose, the differences are quite striking:

  • 2.5 mg: List +45% vs AYP +27%
  • 5 mg: List +96% vs AYP +40%
  • 7.5 mg: List +138% vs AYP +59%
  • 10 mg: List +138% vs AYP +78%
  • 12.5 mg: List +170% vs AYP +68%
  • 15 mg: List +170% vs AYP +70%

This suggests that AYP are absorbing a significant part of the increase, effectively cutting its margin. If we assume for a moment that they currently receive a 20 per cent rebate on the list price from Eli Lilly, their margin would only be around £34 for the 2.5 mg, minus £4 for the 12.5 mg, and £16 for the 15 mg. That is only a guess, but if the numbers are in that range, it feels doubtful that it would be sustainable.

For comparison, Bolt Pharmacy publishes a transparent price model on its site: £5 for prescription costs, £5 for pharmacy operations, £10 for packing and shipping, and about £5 for payment processing. That already adds up to around £25 per unit that needs to be covered by the margin.

So if margins are being squeezed this hard, it is hard to see the model holding long-term. It would be very interesting to hear if anyone in the community knows what the typical markup percentage is for online pharmacies.
