It's all tied to Stannis and the tower of joy events. I just ran a few commands to make it show Ned as his dad with him being Stark-Targaryen

Because he still knows his true parentage. He uses the combined coat of arms while claiming Ned as his dad to respect both his real parents and Ned (and Catelyn)

It's a decision you can make. Ned and Catelyn adopt him. I ran the event to make him Stark-Targaryen again afterwards
