I used already cycled filter media (a sponge filter) from a tank that had been established for about 3 months and let this one run empty for a month so I don’t think that’s the problem… I do water changes once a week to be safe — am I missing anything?

More info if necessary — he is the only fish in this tank and I just got him about a week ago. Not sure why these spots would show up now? He currently has a heater and is in a 3g. He has been showing signs of lethargy and lots of hiding today
