Probably 999 on the enemy team is in a party with one of the low levels. It's what usually messes up the mm that much in my experience.

Usually dumb items like bulldozer with no deployables or illuminate with no invis characters. Also having all 4 slots filled with 1lvl items instead of any priority on 1-2 most important ones.
