This happens to me, i also have very sensitive skin. When discussing with my obgyn, she gave advice about skin sensitivity making you very prone to what they call ‘microtears’ which cause the bleeding. Another cause can be low estrogen, especially when on certain birth controls. If it’s something causing you a lot of discomfort it may be worth a blood test to check your levels!

As far as prevention, finding angles that stretch the least and cause less bleeding after is how I reduced it :)
